Our highly qualified professionals bring a variety of skills, experiences and specialties to our Center. Our team members include physicians who are board certified in Family and Internal Medicine MDs, Pediatric MDs, Family Nurse Practitioners, and Physician Assistants, in addition to certified Medical Assistants. All of our professional’s work in a team environment, serving patients of all ages and medical backgrounds and ensuring that we meet each patient’s individual needs. Meet Our Providers
Dr. Rashid Mohiuddin
Dr. Rashid Mohiuddin, Board Certified Family Physician serves as the Medical Director, overseeing all aspects of patient care and chair of Quality improvement/Quality assurance committee. Dr. Mohiuddin has completed his Medical school from India. He has worked in various countries and worked in FQHC for over 11 years.
Dr. Mohiuddin completed his residency in Family Medicine at Howard University. He has over 17 years of experience and is a fellow of the American Academy of Family Physicians. He has been working as a Medical Director at Greater Prince William Community Health Center since 2009. He speaks Urdu and Hindi fluently. During his free time he enjoys playing sports and traveling.
Dr. Camille Horton-Thompson
Dr. Camille Horton-Thompson lives in Manassas with her husband, and 3 children. She enjoys spending time with her family- traveling, going to the movies, as well as attending soccer matches and gymnastics meets. She also enjoys listening to calypso and different genres of music.
She received a Bachelor of Science in Chemical Engineering from Rutgers University, New Jersey and completed her Doctor of Medicine and Residency in Family Medicine, at the RWJ-University of Medicine and Dentistry of New Jersey. In 2017, she received her Master’s in Public Health from George Washington University.
Dr. Horton-Thompson has worked in Virginia since 2003 and hopes to continue working closely with her patients, to help them aspire to and attain their optimal health.
Syed Mahboob, MD
Dr. Syed Mahboob, MD was born in Pakistan where he completed Medical School from Dow Medical University of Health Sciences. He completed his Family Medicine training from Howard University, Washington DC during which he was awarded the honor of Educational Chief Resident for the Residency Program.
After completing his residency he continued his fellowship in Geriatrics at Emory School of Medicine in Atlanta, GA. After working as a Geriatrician he joined the center in 2015 where he’s worked full time at the Manassas location as a primary care physician.
In his spare time he enjoys outdoor activities such as running, biking, and hiking as well as playing chess and poker. He is also a big fan of the latest Hollywood Movies.
Lena Tran graduated from Howard University, where she was on the Dean’s List. She has been practicing as a Physician Assistant in family medicine and urgent care setting for more than the past 7 years. She believes in patient education, preventative care, in-depth evaluation and treatment. She enjoys building long-term relationships with her patients. Her goal is to provide quality, comprehensive, and compassionate medical care. She is fluent in Vietnamese.
Lena Tran enjoys traveling to new places, exploring different cultures, and spending times with her family.
Anh Pham received her Undergraduate Degree in Biology from George Mason University. Upon completion of her study at Howard University in 2011 and Board Certification as a Physician Assistant, she began working at Gastroenterology Associates of Fredericksburg. She joined the Greater Prince William Community Health Center in 2016.
In her free time, she enjoys traveling and spending quality time with her husband, daughter, and son.
Family Nurse Practitioner
Katie Kotopoulos is a Family Nurse Practitioner. She sees adults and children from age newborn to 100. Her undergraduate degree is from The University of Virginia. She then received a second bachelors degree in Nursing from The Johns Hopkins University and then received a Masters in Nursing from Georgetown University in 2003. She has worked at Georgetown University Medical Center and at the United States Embassy in Dhaka, Bangladesh. She lived, worked and travelled for 7 years in Africa and Asia before joining the Center in 2015.
When Katie is not at work, she enjoys reading, running and spending time with her husband and 4 sons.
Zoila Patricia Congleton
Family Nurse Practitioner
Zoila Patricia "Patty" Congleton has been a Family Nurse Practitioner for over 20 years. She grew up in Lima, Peru and Northern Virginia. She graduated with Honors from George Mason University. Patty is bilingual in English and Spanish and is bicultural.
Patty has 4 kids and enjoys being with her family, watching kid’s sports, gardening, nature and traveling.
She also enjoys volunteering as a soccer team manager for three recreational teams and for the Woodbridge Senior High School wrestling team.
